No. It is a membership plan between you and our practice. There are no claims, forms, or approvals.
When can I use it?
Right away. Benefits start the day you join.
Can I add my family?
Yes. Each added family member saves 10% on their membership.
Who qualifies for Perio Complete?
Patients diagnosed with periodontal (gum) disease. We will let you know at your exam if this plan is the right fit.
What if I already have insurance?
The plan is designed for patients without dental insurance. Call us and we will help you decide what makes sense.
How does renewal work?
Your plan runs 12 months. You get an email reminder 45 days before renewal, and you manage auto-renewal in your member portal.
Can I cancel?
Yes, anytime. See the cancellation terms in the disclosures below.
Disclosures
- This dental plan is not insurance.
- This plan is not a qualified plan under the ACA.
- The plan must be current in order to receive included or discounted services.
- The plan runs for 12 months. You will receive an email reminder 45 days prior to the renewal date. Auto renewal is managed through your member portal.
- You may cancel your Plan membership at any time. You will receive a full refund of membership fees paid to Clerri if (i) the cancellation request is received within the first 30 days of the current annual contract term and (ii) no discounted dental services have been provided to any member(s) during the current term.
